PDSA Team worksheet

PDSA stands for Plan Do Study Act cycle.

It is a useful tool for documenting and planning an improvement by focusing on specific changes.

Fill out one PDSA worksheet for each test you conduct. Your team will test several different changes, and each change will go through several PDSA cycles.

Keep a file (either electronic or hard copy) of all PDSA Worksheets for all changes your team trys out.

Measuring Success

Key performance indicators, or KPIs, are an important element to identify at the beginning of a project so you can plan where you want to be,  keep track of performance so you can see how your project is progressing.

Other ways of measuring success can include capturing the impact on practice, services and quality, exploring trends in key dashboard data and also gathering the perceptions of key stakeholders through for example using claims, concerns and issues and emotional touchpoints
